You’ve already tried to fix it.
But it’s still there.
Maybe you’ve rested it. Stretched it. Iced it. Done the exercises. Had massage. Seen another physio. Maybe you’ve even tried an injection.
Or it improves for a while, then flares up again as soon as you start walking, running, lifting or training.
So the question isn’t: “What else can I try?”
It’s: “Why hasn’t this healed?”
That’s what we want to help you work out.
Less pain isn’t the same as being ready to go again.
Your symptoms may settle before you’ve rebuilt the strength to handle running, lifting or everyday life. The exercises may need to progress. Or we may need to reassess what’s causing the pain.
That’s why we assess it, measure what matters, start treatment and build towards what you want to get back to.
A DIFFERENT APPROACH
This isn’t another “rest and do these exercises” appointment.
Persistent tendon problems can need a different approach. At Premier’s Tendon Clinic, we bring together:
PhD-led tendon expertise
Our physiotherapy team is led by Sanam Tavakkoli Oskouei, Physiotherapist (PhD), whose doctoral research at Monash University included Achilles tendinopathy, physical activity and load.
Shockwave Therapy
A treatment option for stubborn tendon problems that haven’t responded to the usual approach.
Hands-on treatment
Soft-tissue massage, manual therapy, dry needling and other treatment selected by your physiotherapist.
Objective strength testing
AxIT technology can help us measure strength and capacity, identify weaknesses and track progress.
Progressive rehabilitation
Getting the pain down is only part of the job. Your tendon also needs to handle what you want to get back to.
Treatment on day one
Every first tendon appointment includes appropriate treatment selected after your assessment.
WHEN THE USUAL APPROACH HASN’T WORKED
This is where Shockwave Therapy can make a difference.
Tendons can be slow and stubborn to heal. Once tendon pain has been there for months, simply resting it, stretching it or doing the same exercises may not be enough.
Shockwave sends acoustic waves into the problem area. It may stimulate biological activity involved in tendon repair and remodelling.
Think of it as giving a stubborn tendon a new reason to respond.
That’s why we often consider Shockwave when someone tells us: “I’ve rested it. I’ve stretched it. I’ve done the exercises. I’ve already had physio. I’ve even tried injections. And it still hasn’t gone away.”
That’s the type of problem we want to see.
Premier has used Shockwave Therapy since 2016 as part of our approach to persistent tendon problems. Shockwave is available at Brunswick, Fitzroy North and Thornbury.
If your physiotherapist believes Shockwave is right for you, we can start it on day one. You don’t need a separate Shockwave consultation and you don’t need to decide which treatment you need.
We assess it. We choose the right treatment. We start on day one.

Assess it. Measure it. Treat it. Build it back up.

01 — Assess
We look at your symptoms, what you’ve already tried and what you want to get back to.
02 — Measure
We assess movement, strength and capacity. Where useful, AxIT gives us objective measurements.
03 — Treat
Treatment starts on your first visit. That may include hands-on treatment, dry needling, Shockwave or another appropriate approach.
04 — Rebuild
We progressively build strength and capacity rather than simply waiting for the pain to disappear.
05 — Get back
Running. Gym. Sport. Work. Walking without pain. Sleeping comfortably. Whatever your tendon has been stopping you from doing.
That’s the goal.
You will get treatment on your first appointment.
Not just an assessment and an exercise sheet.
We need to work out what’s going on first. Then we start treatment.
Depending on what your physiotherapist finds, that may include hands-on soft-tissue treatment, massage and manual therapy, dry needling, Shockwave Therapy, other appropriate physiotherapy treatment, exercise and progressive loading.
You don’t need to know what treatment you need.
That’s our job.

PhD-led tendon care
“But I’ve already tried physio.”
Good. Tell us exactly what happened.
We don’t want to ignore everything you’ve already tried and start from scratch. We want to know what helped, what didn’t, whether the pain went away, whether it returned when activity increased, and what stopped you getting back to normal.
If the old approach hasn’t worked, repeating it probably isn’t what you’re looking for.
We want to work out what needs to be different.
IS THIS FOR YOU?
Persistent tendon problems we see
You don’t need to diagnose yourself. Start with the pain that is changing how you walk, run, lift, train, work or sleep.
Achilles pain
Pain and stiffness affecting walking, running or sport.
Heel pain
First-step pain, pain after sitting or pain under the foot.
Knee pain
Patellar tendon pain with stairs, squatting, running or jumping.
Outer hip pain
Gluteal tendon pain affecting walking, running or sleeping on your side.
Shoulder pain
Pain affecting reaching, lifting, gym, work or sleep.
Elbow pain
Tennis elbow and golfer’s elbow with gripping or lifting.
Not sure if it’s actually a tendon? We’ll work that out.

Questions that keep people waiting
Will I get treatment on the first day?
Yes. Every first tendon appointment includes appropriate treatment. We assess the problem first, then start the treatment your physiotherapist believes is most appropriate.
Will you do hands-on treatment?
Hands-on treatment is one of the options our physiotherapists use. This may include massage, soft-tissue treatment, manual therapy or dry needling.
Can I get Shockwave on the first day?
Yes — if your physiotherapist believes it’s appropriate for your problem. Shockwave is available at all three Premier clinics.
I’ve already done exercises. What will be different?
Doing exercises isn’t the same as progressively building your body to handle the activity you want to return to. We assess where you are now and build towards where you need to be.
Do I need a scan?
No. Bring any scans or reports you already have. If further investigation is needed, we can discuss that with you.
Do I need a referral?
No. You can book directly with any Premier physiotherapist.
How much longer are you going to wait for it to heal?
You’ve rested it. You’ve stretched it. You’ve tried the exercises. Maybe you’ve already tried physio or injections. And it’s still there.
You can keep hoping it eventually settles. Or you can find out why it hasn’t improved and what needs to happen next.
